This form does not accept maintenance requests. If you call the office about maintenance, you will be transferred to the Maintenance Department (785-307-3040).
For all other tenant requests, please review the following before submitting:
- If you call the office regarding any request or question, you will be directed to use this form.
- This form is for tenant requests only. Submission does not guarantee approval or action.
- You will receive a status update — not a final resolution — within 2 business days.
- Do not proceed with any action related to this request until you have received written approval from NextHome Unlimited. Action taken without written approval is considered a violation of your lease agreement.
- All agreements must be documented in writing and signed by both you and NextHome Unlimited to be valid.
- All communication regarding this submission will occur via email.

Fair Housing accommodation. Service animals and ESAs are reasonable accommodations under the Fair Housing Act. The pet fee and pet rent do not apply.
For ESAs: a letter from a licensed mental health professional. For service animals: training documentation (if applicable).
Important: Unauthorized occupancy changes will be treated as a violation of your lease agreement and may result in fees or legal action. Please submit this request before anyone moves in or out.
Adding a tenant — what to expect. The proposed occupant must complete a full rental application, pay the required application fee, and pass a 5-point background and credit check. You'll receive the application link from us within 2 business days of this submission. Upon move-out, the Security Deposit will be issued jointly to all tenants listed on the lease at that time, regardless of individual financial contributions. Removing a tenant — what to expect. The departing tenant must agree to and sign off on the removal. The departing tenant remains financially responsible until the lease amendment is signed by all parties. The Security Deposit remains with the property and will only be issued back to the tenants remaining on the lease at the time of move-out. Full policy: Adding or Changing Roommates and Occupants.

Property Modifications policy. Before making any change to the property — no matter how small — you must receive written approval. Unauthorized modifications are a lease violation and may result in charges at move-out (vendor invoice + 25% admin fee). Full policy: Property Modifications and Improvements.

Payment Agreement Policy. All payment agreements must be completed within one (1) month and require an Initial Installment Payment, due within one business day of the agreement being signed by all parties. You may structure the agreement into 2, 3, or 4 installment payments (in addition to the Initial Installment).
Timing & fees: Payment plan requests must be received on or before the 3rd of the month. Requests received after the 3rd are considered late and will incur a one-time $35 payment plan fee. Late fees continue to accrue until the agreement is signed by all parties.
Failure to fulfill the terms of the signed agreement will result in all waived or deferred late fees being retroactively applied to your account in accordance with your lease agreement. All agreements must be documented in writing and signed by both parties to be valid.
Note: payment agreements count toward the late-payment limit for renewal eligibility (max 4 late payments / payment plans in any 12-month period). Related: Late Fee Policy.

Forwarding address*
SCRA / PCS process. Active-duty members may terminate early under SCRA. Termination becomes effective 30 days after the next full rental payment is due, after we receive your Notice to Vacate + valid official orders. Full policy: Military Orders and Early Lease Termination (SCRA / PCS).
Renewal eligibility. Renewal is considered when your account shows strong compliance over the last 12 months: ≤2 NSF fees, ≤4 late payments or payment plans, no lease violations, and the Annual RentCheck Inspection completed by deadline. Heads up — Insurance. NHU requires proof of renter's insurance on file in your tenant portal. If we don't have valid proof, we may place the home into Liability to Landlord Insurance (LLI) and the fee is system-applied to your account each month it's missing — automatic and non-refundable. Update your insurance info in your AppFolio tenant portal to stop the fee. Full policy: Renter's Insurance Requirement.
Heads up — Utilities. You are responsible for utilities, mowing, and property upkeep through your lease end date — not just through your move-out date. Stopping services or leaving the property unmaintained before your lease ends may result in charges or a lease violation. Full policy: Utilities Setup and Requirements.
Heads up — Late fees. Rent is due on the 1st; late fees start at $50 on the 4th, plus $25/day from the 5th onward until the balance is paid in full. Late fees stack. NSF (returned payment) fee is $50 per occurrence. Full policy: Late Fee Policy.
